Graduation Requirements

  • FULLY ACCOMPLISHED CLEARANCE. No  blank entries
  • Certificate of Candidacy (CoC)
  • Photocopy of All Receipts (if any)
  • Evaluation Result, NSTP form
  • Information Data Sheet

Only complete requirements will be entertained.

Printing of Clearance, CoC, NSTP, Evaluation Sheet, Information Sheet will be on “August 28 up to September 6” only. This is the last date requested. No extension.
